RN Clinical Hospice Liaison 

Role Overview

A registered nurse–based liaison role focused on building relationships with hospitals, physicians, and healthcare facilities to increase hospice referrals and support patient access to hospice services. The role combines clinical expertise with outreach, education, and relationship development.

Schedule & Location

  • Monday to Friday, 8:00 AM – 4:30 PM
  • Mesa, Arizona (field-based hospital and facility outreach)

Key Responsibilities

  • Develop and maintain relationships with physicians, hospitals, discharge planners, case managers, and care facilities
  • Identify key decision-makers and strengthen referral networks
  • Promote hospice services and educate healthcare partners on care options and eligibility
  • Conduct patient and family discussions about hospice philosophy and services
  • Support patient eligibility assessments when needed
  • Drive referral growth and maintain census targets through consistent outreach

Requirements

  • Active, unencumbered Registered Nurse (RN) license
  • Bachelor's degree in Nursing or healthcare field preferred
  • Minimum 1 year healthcare sales/marketing experience
  • 1–2 years hospice or palliative care exposure strongly preferred
  • Strong relationship-building and communication skills
  • Valid driver's license, reliable transportation, and insurance

Compensation

  • Estimated salary range: $95,000 – $105,000 annually (based on experience and role scope)
