Job Title: Social Worker - Diversion Specialist
Location: Norwalk, CT
Salary $52-54K+/year depending on experience
About the Role
We are seeking a motivated social service professional to help prevent homelessness by working directly with individuals and families in crisis. This role focuses on creative problem solving and short-term case management to identify safe housing alternatives and avoid shelter entry whenever possible.
Key Responsibilities
Conduct initial assessments (CAN appointments) for clients entering the system
Work with clients to identify immediate housing solutions outside of shelter
Provide light case management and develop housing stabilization plans
Connect clients to community resources and basic needs (food, clothing, etc.)
Collect and enter required documentation into HMIS
Collaborate with community partners, landlords, and service providers
Maintain accurate records and participate in team meetings and trainings
Qualifications
Bachelor’s degree, 3+ years of related experience, or relevant lived experience
Strong problem-solving and communication skills
Ability to work independently and as part of a team
Knowledge of homelessness, mental health, or social services preferred
Valid driver’s license and reliable vehicle required
